2. Use block letters only to complete the form.
3. The form must be accompanied by copy of National ID, copy of latest pay slip and two (2) passport size photographs. One photograph will be returned to you with your Kenyatta Matibabu SACCO Membership Card.
4. Share contributions not less than 2000/= or 5% of basic salary whichever is higher.
5. Complete the attached nomination/next-of kin form, and return the same in a sealed envelop with your name and P/NO. addressed on the envelop.
